Organizations must complete an application (i.e. Pantry Partner Agreement) and submit it to MAFB for review and approval before they can receive food distributions. Once the application has been reviewed, the organization will receive an email from MAFB approving the application or requesting additional information before approval can be granted. Applications may be denied if the organization is not located in the designated service area or the organization cannot ensure that it can maintain the food safety and storage requirements as outlined in the Agreement.
Applications can be emailed to pantrypartners@mafbfeedingsc.org or mailed to Midlands Area Food Bank, 720 Old Clemson Road #D-1, Columbia, SC 29229.
For organizations and churches that are not located in a designated service area, they may submit an application but will need to provide documentation that they are serving families in the service area (currently Kershaw, Sumter, and Lee Counties) before being approved.
An application must be completed for each household that is served. Pantry Partners should maintain a copy of the application for their records, which the individual will need to sign each time the household receives food. Please note that due to Covid and restrictions that may be in place at pantry sites, a pantry volunteer or staff member may also sign the application on behalf of the individual. After signing for the individual, the volunteer or staff member should write his/her initials beside the signature. Also, MAFB may periodically request to review the applications.
Pantry Partners should use this information when determining if households meet the income guidelines to receive TEFAP food distributions. In addition, partners may also distribute this guidance sheet to households for their reference.
These forms are required to be disiplayed at your pantry site.
